Aspirin: Aspirin is ineffective in preventing heart disease when the blood pressure is high. Studies did find that aspirin did help to prevent both heart attacks and strokes, but only in men who were at the lowest risk to begin with. The overall reduction of heart attacks as a result of taking aspirin was 20%, but this was greater in younger men (below 65) and there was no apparent benefit in men over 65. And in men with low systolic pressure (less than 130 mm Hg), there was a reduction of nearly 50%, while it was 25% when the pressure was between 130 and 145, and there was no effect if it was above 145. Aspirin also reduced strokes, but again this was only in men with systolic pressures below 145 mm Hg.

Tobacco: Avoid tobacco products and secondhand smoke. On top of other health threats, the nicotine in tobacco products can raise your blood pressure by 10 mm Hg or more. Although that increase may be only temporary, lasting 30 to 60 minutes after using tobacco, having repeated elevations through the course of the day means your blood pressure may remain constantly high. In addition, chemicals in tobacco can damage your arteries and cause fluid retention, both of which can raise your blood pressure. And like alcohol, tobacco products can interfere with the effectiveness of your blood pressure medications. Don't use any tobacco.

Goal: The goal in treating hypertension is to reduce the risk of serious complications, including heart disease and stroke. While the optimum blood pressure is 120/80 mm Hg, even partial reduction in blood pressure is beneficial. Prescription medications are often needed to treat hypertension, but lifestyle modifications, including diet, exercise, and relaxation, is necessary with or without medications. In fact, a National Institute of Health (NIH) statement issued in 1996 asserts that behavioral and relaxation therapies must be integrated into conventional medical treatment of high blood pressure. Examples of relaxation techniques include biofeedback, massage, meditation, and qi gong. Often, in the early stages of hypertension when blood pressure elevation is mild, the doctor will recommend lifestyle modifications alone for a period of 6 to 12 months. After this time, if blood pressure is still high, medication will likely be prescribed.

Salt: Add less salt at the table and in cooking. Reduce the amount a little each day until none is used. Try spices and herbs instead. Use fewer sauces, mixes, and "instant" products this includes flavored rice, pasta, and cereal, which usually have salt added. Limit smoked, cured, or processed beef, pork, or poultry. Buy fresh, plain, frozen, or canned, "with no salt added", vegetables. Use fresh poultry, fish, and lean meat, rather than canned or processed types. Use herbs, spices, and salt-free seasoning blends in cooking and at the table. Cook rice, pasta, and hot cereals without salt. Cut back on instant or flavored rice, pasta, and cereal mixes, which usually have added salt. Choose "convenience" foods that are lower in sodium. Cut back on frozen dinners, pizza, packaged mixes, canned soups or broths, and salad dressings -- these often have a lot of sodium. Rinse canned foods, such as tuna, to remove some sodium. When available, buy low- or reduced-sodium, or no-salt-added versions of foods. Choose ready-to-eat breakfast cereals that are lower in sodium.

Alcohol: Observational studies and clinical trials have documented a direct, dose-dependent relationship between alcohol intake and BP, particularly as the intake of alcohol increases above 2 drinks per day. Importantly, this relationship has been shown to be independent of potential confounders such as age, obesity, and salt intake. Although some studies have shown that the alcohol-hypertension relationship also extends into the light drinking range (2 drinks per day), this is the range in which alcohol may reduce coronary heart disease risk. A recent meta-analysis of 15 randomized controlled trials reported that decreased consumption of alcohol (median reduction in self-reported alcohol consumption, 76%; range, 16% to 100%) reduced systolic and diastolic BP by 3.3 and 2.0 mm Hg, respectively. BP reductions were similar in non-hypertensive and hypertensive individuals. Importantly, the relationship between reduction in mean percentage of alcohol and decline in BP was dose dependent. In aggregate, available evidence supports moderation of alcohol intake (among those who drink) as an effective approach to lower BP. Alcohol consumption should be limited to 2 alcoholic drinks per day in most men and 1 alcoholic drink per day in women and lighter-weight persons. Note that 1 drink is defined as 12 oz of regular beer, 5 oz of wine (12% alcohol), and 1.5 oz of 80-proof distilled spirits.
